Administrative Procedure

In filing claims under the scholastic accident plan with the Approved Carrier, the following procedure is advised:

    1. The person in charge of claims at the school should complete a claim form upon knowledge of a claim and make a copy for the school's record.

    2. The claim form is given to the child with instructions to take the form to the Garrett County Memorial Hospital insurance department.

    3. The insurance department will then attach the hospital bill to the claim form and put it in the doctor's mail box so that he can complete the attending physician's
        statement, attach his bill to the claim form and then forward the form and bills to the claims department of the Approved Carrier.

    4. When a claim occurs and the Garrett County Memorial Hospital is not involved the child should be instructed to take the claim form to the doctor with the hospital
        bill attached to the claim form. The doctor will then complete the attending physician's statement, attach his bill to the form, and mail the claim with all bills to
        the claims office of the Approved Carrier.
