SAMPLE LETTER

Dear Parents:

As part of the Public Schools's health screening program, scoliosis screening will be provided for all 6th and 8th grade students during the coming weeks. Scoliosis is a lateral curvation of the spine, and screening involves a visual examination of the spine by a trained examiner. Early detection and treatment (where indicated) can prevent later health problems and need for surgical correction.

If your child is currently under a doctor's care for scoliosis or you prefer not to have your child screened, please notify the school in writing.  Only those students with a signed letter will be excused from the screening.

Sincerely,

(Adm. Procedure 472.61 - 7/10/86)
